Une attestation d’aptitude médicale valide délivrée en application de la réglementation fédérale applicable sauf si une mention y relative figure sur le permis de conduire du candidat. Si le candidat chauffeur est un ressortissant étranger, le document l’autorisant à travailler en Belgique. Une photo d’identité du candidat chauffeur (sur fond blanc).
